MOBILE Check In has arrived!!!
We are excited to introduce our new Pre-Check in process. No more filling out forms in the waiting room. Once you have scheduled an appointment, you can complete your demographic, insurance and medical information in advance on your own device.
It’s simple – here’s how it works: Before your appointment, you will receive an email and/or text message. Please choose one method to complete your appointment registration Pre-Check in. During the pre-check in please make sure to follow all screen prompts and complete all questions. If not, you will be asked the same questions at the time of your appointment. When you arrive at our office for your appointment, please sign in on the check in clipboard and have a seat in our waiting room. If you do not receive a pre-check in link or do not have the ability to pre-check in, you are invited to print and complete the following forms (PDF) prior to your appointment, and turn in upon checking in.
Health Insurance Information
We are participating providers with Medicare, most Medicaid, and commercial insurance plans.
What if your plan isn’t on our list?
We recommend that all patients check with their benefits representative at their health plan prior to seeking care. We will always do our best to maximize the health plan benefits you have. If your medical insurance requires a referral, you may need to contact your Primary Care Physician to let them know when your scheduled appointment is and ask them to submit a referral for you.
Requests for Medical Records
We will obtain copies of your medical records from your referring provider / primary care provider. Our patients can now request and download complete health records of services and visits done at Waco Cardiology online through Healthmark Group, our partner for providing medical records. Waco Cardiology Associates and Healthmark Group are pleased to waive all copying fees for Waco Cardiology Associates patients.
How?
Requests may be submitted electronically to HealthMark’s Request Manager at https://requestmanager.healthmark-group.com. Once logged in, select “Submit Request” from the menu options and enter all required fields to submit your request. Your medical record request will be processed, and a notification will be sent via mail or email once complete and available for download.

Outside Requestors Providers, STAT, and urgent request requests for records should be directly faxed/mailed to *insert clinic name*. Sending these requests to Healthmark can cause the request to be delayed. If you are sending records to us, these should also be sent *insert fax/mailing information*.
